Definition: Telehealth is the consistent and accurate remote monitoring of patients' vital signs usually from their own home.
 

How does telehealth work?
 
1.       Patients take their vital signs from the comfort of their own home, using the Tunstall mytelemedic telehealth system.
2.       Collected data is transmitted to a triage centre where it is received by the icp triagemanager software. icp triagemanager sorts the data and prioritises patients on a needs and severity basis. Action is taken in accordance with the patient’s care plan, such as community nurse to attend or GP or specialist notified.
3.       Clinical reports are provided to the patient’s care team for ongoing assessment.
4.       Data is assessed for follow up, allowing proactive case management, accurate medication prescription and immediate intervention.
